Our Policies
Our office policies ensure a smooth experience: new patients are scheduled with the first available physician unless specified otherwise, and follow-ups are with the chosen doctor unless an emergency arises. We require prior authorization from primary care physicians and request 48 hours’ notice for appointment cancellations and 72 hours for procedures. Co-pays and deductibles are due at the time of service. We accept various payment methods and work with multiple insurance plans, but please confirm coverage with your provider. Office hours are Monday to Friday, 9 AM to 5:30 PM, with a 15-minute appointment window; late arrivals may need to reschedule.
Appointments
New patients will be scheduled with the first available physician unless requested otherwise by you or the referring physician. The office staff will try to accommodate your request to see one of our doctors at a convenient time and office location. However, in case of an emergency and if your physician is not available, you may be seen by the on-call physician.
Authorization
We are specialist providers of Physicians Medical Group of San Jose (PMG), Santa Clara County IPA (SCCIPA), Premier Care Medical Group, Valley Health Plan, Affinity Medical Group and Northern California Physicians Network (NCPN). We are required to obtain authorizations from primary care physicians prior to an office consultation. Subsequently, we can initiate authorization requests for patients for follow-up appointments.
Cancellations
We require at least 3 business days’ notice for cancellation of an office appointment and 7 days’ notice for cancellation of any procedure. A cancellation fee will apply if you do not provide this notice or fail to keep your scheduled appointment or procedure.
Finances / Insurance Plans
Co-pay and deductibles are collected at the time of service provided as required by California Law Business Professional 1680U. Installed payments may be considered on a case-by-case basis and must be arranged in advance. We take American Express, Discover, MasterCard and Visa and Cash.
Contracted Health Plans:
San Jose Gastroenterology physicians are providers of Physicians Medical Group of San Jose (PMG), Santa Clara County IPA (SCCIPA), Premier Care Medical Group, Valley Health Plan, Affinity Medical Group and Northern California Physicians Network (NCPN). We accept most PPOs, Tricare, and Medicare.

Labs, X-Rays & Facilities
We make the best effort to send you to the laboratories and facilities that are contracted with your insurance. However, insurance plans often change and alter their policies and networks. You are solely responsible for your own health insurance and payments. Our office only has a professional relationship with these ancillary service facilities. We have no control and cannot be responsible for their policies, services and charges. Please check with your insurance policy.

Our affiliated facilities are all conveniently located near our three San Jose clinics. This proximity makes commuting easy for patients, allowing them to receive high-quality care close to home.
