New patients will be scheduled with the first available physician unless requested otherwise by you or the referring physician. The office staff will try to accommodate your request to see one of our doctors at a convenient time and office location.
Follow up appointments will be made with your chosen doctor. However, in case of an emergency and if your physician is not available, you may be seen by the on-call physician. You can subsequently follow up with your regular physician for continuity of care.

We are specialist providers of PMG, SCCIPA, and Premier Care.
We are required to obtain authorizations from primary care physicians prior to an office consultation.
Subsequently, we can initiate authorization requests for patients for follow-up appointments.
We request 48 hours notice of cancellation of an office appointment and 72 hours for cancellation of any procedure.
A cancellation fee is applied if you do not notify our office with this notice or fail to keep your scheduled appointment/procedure.

Co-pay and deductibles are collected at the time of service provided as required by California Law Business Professional 1680U.
Installed payments may be considered on a case-by-case basis and must be arranged in advance.
We take MasterCard/Visa and American Express.
San Jose Gastroenterology physicians are providers of Physicians Medical Group of San Jose (PMG), Santa Clara County IPA (SCIPA), and Premier Care Medical Group and Valley Health Plan. We accept most PPOs, Tricare, and Medicare.
We also offer special cash discount prices for cash-paying patients for our professional fees and at our associated ambulatory surgery centers.

We make the best effort to send you to the laboratories and facilities that are contracted with your insurance. However, insurance plans often change and alter their policies and networks.
You are solely responsible for your own health insurance and payments. Our office only has a professional relationship with these ancillary service facilities. We have no control and cannot be responsible for their policies, services and charges. Please check with your insurance policy.
Hospitals:
Ambulatory Surgery Center:
Physicians prescribe appropriate medications according to the patient’s medical problems. They will also try to prescribe the medications or change to similar medications as approved by the patient’s health plan formulary.
Whether the medications are covered or on formulary is dependent on the contract between the patient and the health insurance plan.

Please contact your local pharmacy for medication refill requests.
Also, please allow our office 48 hours to refill your medication requests. Refills will not be done on weekends or after hours.
Office hours are Monday-Friday 9 AM - 5:30 PM. We respect your time and try our best to keep your appointment time within 15 minutes.
If you are more than 30 minutes late, we may have to reschedule your appointment.
